The Referral Process

Like all hospitals and clinics, Heath Lodge requires a referral for ALL procedures to be carried out.

Referrals are accepted from the following sources:

  • Specialist Clinician
  • GP
  • Chiropractitioner
  • Osteopath
  • Physiotherapist

Referrals should include personal details, including a contact number for the patient and a brief description of the symptoms and the procedure required. A fax number from the referrer is beneficial for the speedy delivery of results.

A referral form is available on this website.  We do accept referrals on other forms, as long as all the required information is given, if we do not have all the information it may delay your appointment. We will only offer appointments if we have received the referral form.


Referrals can be sent by post or faxed to Heath Lodge Clinic (see contact us).


The Booking Process

When Heath Lodge Clinic receives your referral, we will attempt to contact you on the numbers supplied on the referral form, failing that, we will write to you requesting that you telephone us.

We will attempt to offer you an appointment which suits your schedule and normally within 2 days.

We will take any personal details we do not already have and go through a few safety questions with you. It is at this point you will be asked how you are funding your procedure, if you are privately insured we will ask you for your authorisation code (some insurers do not require a code).We will also ensure you know how to find us, and give you details if you do not.

As our appointments are usually within 2 days, we do not send written confirmation, however if you would like this, please let us know at the time of booking. If you have any further questions, worries or concerns, we will do our best to help you.

Please let us know at the time of booking if you have any special requirements examples below:

  • Wheelchair access
  • Hard of hearing
  • In need of an interpreter to accompany you
  • Claustrophobia       

Please let us know as soon as possible if you are unable to attend for your appointment so that we are able to reallocate the appointment to another patient.
